Welcome to Ingraham Athletics!

At Ingraham, athletics are an extension of the classroom. Hard work, dedication, willingness to try new things, and taking risks to expand our mind, body, and spirit are included in all aspects of a comprehensive high school such as ours.

Ram Athletics is committed to being a model program where all athletes are empowered to compete to the best of their ability with the utmost integrity. We believe deeply that what we do, what we say, and how we show up should reflect our School motto: It’s A Matter of Pride.

Fall Sports include: Football, XC (Girls, Boys), Golf (Girls, Boys), Girls Soccer, Girls Slow Pitch Softball, Girls Swim & Dive & Girls Volleyball, and Cheer (Girls) and Boys Ultimate (this is a club sport, so you must register on FinalForms AND DiscNW

Final Forms Sports Registration
Important: Parent/Guardian creates a parent account to give student access to sign forms.
Use the name the student used for school enrollment.
Students must use a personal email (e.g., msn.com, gmail.com). District email will not work.

Sports Physical

  • Must be a document of physician assessment with signature and clinic stamp.
  • Date must be current and valid through the end of the sport season.
  • Physicals are valid for two years; registration forms and fees are due each year.
  • Schedule physical appointments at Ingraham Teen Health Center or local clinics such as Indigo Clinic (206-365-9000).
  • Upload physical documents in PDF format only. Photos will not be accepted.

Medical Insurance

Enter insurance company name and policy or group number in registration.

Special Student Categories

  • Private/Home Schooled Students: Complete WIAA Eligibility Section in Final Forms. Attach June grades and submit semester grades during the season.
  • Running Start Students: Complete WIAA Eligibility Section with required signatures. Upload form in PDF. Athletic Department Secretary can assist with staff signatures.
  • Newly Transferred Students: Meet with Athletic Director. Attach current grades/transcript and previous school form.

Student ID Cards & Fees

  • ID cards issued on picture day in September.
  • ASB stickers available from Athletic Office with proof of sports fee payment.
  • ASB Fee: $75 per year covers three WIAA sports seasons and includes club sports.

  • Club Sport Fee: $75
  • Fee waivers available for Free and Reduced Lunch students. Contact Athletic Secretary at 206-252-3889.

Eligibility Requirements

  • GPA must be 2.0 or higher.
    Fall and Winter sports eligibility is based on previous semester/June grades.
  • Student must be enrolled in at least 6 classes.
  • Student should be on track to graduate with enough credits.
  • Grade checks occur every 5 weeks.
  • School attendance is required for practices and competitions.

FinalForms Reminders

  • Starting August 11, 2025, families must be enrolled with Seattle Public Schools to have an active FinalForms account
  • Athletic forms are valid for one school year only
  • We are using FinalForms to register online for high school athletic programming. If you previously had an account, you can log in and all your info will still be there!

    If you have not created an account, please follow the instructions below. A parent/guardian must create their FinalForms account first, before the student is able to create their account.

    To accurately complete your student’s account, it will be helpful to have the following information available to complete your student’s online registration:

    • Health Insurance Information (Company and Policy Number)
    • Basic Medical History and Health Information
    • Doctor and Dentist Contact Information
    • Physical Paperwork (WIAA Exam Form Required)

    Parents/Guardians

    1. Go to the SPS FinalForms website
    2. Select “Click Here for a Confirmation Email”
    3. Check your email for a confirmation link, and click confirm your account in the email text.
      • You will receive an email within 2 minutes prompting you to confirm and complete your registration. Remember to check your spam folder.
      • Reminder: FinalForms will use the same parent/guardian email on file in PowerSchool
    4. Create your new FinalForms password and click confirm account.

    Students

    1. Students will use their district (SPS) email account to log into FinalForms
      • Reminder: Students new to the district (or participating as Athletics Only) should contact their school for log-in support

    Physicals are valid for 24 consecutive months.

    • The History document (Page 1) should be completed and remain with your provider
    • The Exam document (Page 2) should be completed and remain with your provider
    • The Medical Eligibility document (Page 3) should be the only item submitted to your school

    Only the WIAA Physical Participation Form (linked above) is accepted.

    The FinalForms platform has a dedicated section to upload a completed preparticipation physical MEDICAL ELIGIBILITY form.

    Student-athletes must have health insurance to play a sport. If not covered by a parent/guardian’s insurance plan, insurance can be purchased through SPS.

    SPS Student Insurance Enrollment

    If families are unable to access this online platform, they should contact their high school athletic director and/or athletic support staff. Computers, as well as physical packets for uploads, can be made available for families to complete registration online.
