Welcome from the Health Office
Dear Parents, Guardians, and Students,
Welcome to Ingraham High School!
My name is Janet Boyett, and I am your school nurse. In addition to school nursing, I have a background in the ICU caring for patients with traumatic injuries, burns, and bone marrow transplants—and on the lighter side, I’ve spent many summers as a camp nurse.
I work Mondays through Wednesdays and share the nurse position with Anna Chamberlin, who is here Thursdays and Fridays. Anna brings a strong background in psychiatric nursing and mental health support.
Our job is to support your child’s health and learning. While we hope to connect with you in person, we’re always available by phone or online to answer questions, handle health forms, and provide guidance.

Annual Health Information
Please provide updated health and medication information for your child before the first day of school. You can complete the Annual Student Health Update online in PowerSchool (Sept. 1–30) or by contacting us directly.
If your child has a life-threatening condition such as diabetes, seizures, or severe allergies, please ensure we have current care plans and medication authorizations on file. We request these be submitted by Sept. 3 if possible. Providers may fax paperwork directly to our confidential fax line.
Ingraham Teen Health Center
Separate from the school nurses, the Ingraham Teen Health Center (ITHC) is a free, confidential health clinic for all Ingraham students. Services include:
- Well-child exams & sports physicals
- Immunizations & flu shots
- Mental health counseling
- Reproductive health care
- Prescriptions for chronic conditions
The ITHC is operated by Public Health and is located next to the attendance and counseling offices.
Registration: Students must register before receiving services. Parent/guardian signatures are required for most services, and the registration form is available online or in person. Exceptions: For mental health and sexual health services, minors can self-consent under Washington state law.
The Teen Health Center is free to students and families, and all SPS students can access other school-based health centers across the district. To make an appointment at the ITHC, call 206-477-9715.
Immunizations
Some vaccines are mandatory for school attendance. We will contact you if your child is missing required immunizations, but it is possible we may not have all of your child’s vaccine records on file.
In addition to required vaccines, the Centers for Disease Control and Prevention (CDC) recommends that teenagers receive: Hepatitis A (2 doses), Meningococcal B (2 doses), Meningococcal A (1 or 2 doses), HPV (2 or 3 doses).
These vaccines are not required for school but are strongly recommended for teen health. All are available through school-based health centers or your healthcare provider.
Strengths and Difficulties Questionnaire (SDQ)
We may use the SDQ screening tool in 9th grade to help identify students who could benefit from additional academic or social support. If you do not want your child to participate, please contact us for an exemption form.
Medications at School
If your child will need medication during the school day, we must have an Authorization for Medication form on file. Please return forms as soon as possible, even if we are not yet in full in-person learning.
